Hearts & Minds: Data Privacy Policy: May 2018

 

As a consequence of the running of Hearts & Minds and in connection with the services provided (including our website) we may obtain certain information about you such as addresses, emails and telephone numbers.

 

This Data Protection Privacy Policy sets out the principles governing our processing policy and use of that information.

 

By signing the Hearts & Minds’ membership form providing information about yourself such as your name, address, telephone numbers and e-mail addresses you agree to our processing, storing and using that information in accordance with the principles set out in this Privacy Policy.

Principles

We limit the collection and use of your information. We only collect such information directly from you, as is required to provide to you with the services, up to date information and activities as stated in the Hearts & Minds Objectives:-

 

  1. To increase awareness of heart disease, its causes, treatments, implications and help available.
  2. To provide social and personal contact with people who share and understand their needs and problems.
  3. To liaise with existing groups, organisations and institutions in this and related fields.
  4. To promote and provide support on aftercare for patients, partners, and carers when resuming home and social life.

 

We will not disclose any details we hold about you to any third party other than as required by law.

 

We will always maintain control over the confidentiality of your information. If you have consented, by registering on the website (www.basildonheart.org.uk) or other means, we may periodically send you information relating to the Hearts & Minds and other carefully selected third parties. If at any point you want to change your preferences or need to review and update your personal details you should contact the Data Officer via our web contact page.

 

You have a right of access under the Data Protection Act 1998 to see the information we hold about you on our records.

 

When we collect information about you we make sure that we minimise what we collect.

 

We try not to keep your information for longer than we need it and we make sure that the information we have about you is manged properly and deleted promptly and securely when we no longer need it.

 

We will hold information collected by us on computer and in other forms. This information may be used for the purposes disclosed in this Privacy Policy.

 

Hearts & Minds reserves the right to monitor and intercept communications for lawful purposes relating to the services provided.

 

We will take all reasonable steps to safeguard the security and confidentiality of any information you provide to us.

 

We cannot however guarantee the security of any information you disclose.

 

You accept the inherent security implications of providing information and will not hold us responsible for any breach of security or disclosure of information unless we have been proved to be negligent.

 

We make every effort to make sure that emails we send you are free from viruses, this cannot be guaranteed, and we recommend that you scan all emails for viruses with appropriate and frequently updated virus checking software.

 

We will not reveal any of your information to any external organisation unless we have previously informed you that this will happen through disclosures or agreement, and we have your authority to do so. We can also disclose your information to authorised parties if we are required to do so by law.
